- Obtained by Black Marrow Reliquary (since the chest rare item drops were nerfed it is very very rare to obtain an orphanage portal gem).
- Destination: The Orphanage
- Does not stack.
- During the Cloak of Darkness event, the portal gem was added as a random reward at the High Stakes Casinos.
